Position Summary:

Clinical Pharmacy Liaison works closely with patients, providers, and pharmacy staff to ensure smooth, consistent access to medications, supporting patients who are seen by the Clinical Pharmacy team and helping patients overcome barriers to fill their medications to improve outcomes and experiences.

Key Responsibilities

Support the clinical pharmacy team in daily operations.

Schedule and help prepare patients for clinical pharmacist appointments.

Assist with triage of Clinical Pharmacy referrals.

Receive referrals or warm hand-offs to assist patients with medication access.

Assist with prescription processing and billing issues, including prior authorizations.

Advocate for patient access and resolve medication‑related access issues; identify and coordinate Patient Assistance Programs (PAP).

Review adherence reports and contact patients to identify and address barriers to medication use; navigate the health care system to connect patients to other health team members.

Conduct medication histories.

Respond to provider questions within the scope of the technician's practice.

Help manage patients in medication synchronization and adherence programs.

Act as a liaison across NeighborHealth pharmacy teams and clinical departments.

Contribute to pharmacy program projects and quality improvement initiatives.

Support overall pharmacy program as needed.

Work in a functional team and receive direction from multiple people, including pharmacists and other team members.

Required Qualifications

PTCB Certified Pharmacy Technician.

Advanced Certified Pharmacy Technician (CPHT‑ADV) or obtain within 90 days of hire.

Certificates in Regulatory Compliance, Billing and Reimbursement, Medication Therapy Management, Medication History.

Registered with the Massachusetts Board of Pharmacy.

Minimum of 3 years of experience in pharmacy or related field.

Proficient in Microsoft Office (Excel, Word, Outlook).

Strong communication skills.

Preferred

Fluent in Spanish.

Experience with Epic and Willow.

Familiarity with 340B pricing.

Strong organizational skills and attention to detail.

Empathetic, patient‑focused, team‑player.

Comfortable with multitasking and adapting to new workflows.

Self‑driven and able to prioritize tasks.

Details

Position Type: Full‑time, on‑site; typical schedule Monday‑Friday, evening and weekend support may be required based on Pharmacy Department needs.

Location: 10 Gove Street – Taylor Building and 20 Maverick Square – Cradock Building.

Hourly Rate:

Starts at $21.00 per hour, up to $30.00 per hour based on experience.
