Position Overview We seek an experienced, self‑driven Physical Therapist (PT) to deliver exceptional care across the region. As a PT, you will evaluate patients, develop individualized treatment plans, and work to improve mobility, strength, and quality of life, collaborating closely with the Clinical Director and interdisciplinary team.
Key Responsibilities
Provide physical therapy services within scope of care, following patient’s individualized plan in coordination with Interdisciplinary Team.
Administer treatments, monitor patient conditions, and report any changes to Physician, Case Manager, Director of Nursing as needed.
Maintain accurate clinical records and submit documentation according to company policy using company‑provided device.
Comply with all company policies and procedures to ensure high standards of care.
Requirements
Current Physical Therapist (PT) License to practice in California (Required).
At least 1 Year experience in Home Health or Hospice care (Required).
Current California Driver’s License.
Able to travel within a 15‑25 mile radius on a regular basis.
