Purpose The Jewish Board’s Community Care Management programs provide compassionate, high‑quality, evidence‑based services to individuals and families in the communities we serve. Our staff use a culturally competent, person‑centered approach to help individuals and their families develop skills and resources to improve overall functioning, instill hope, and strengthen resilience.

Position Overview Care Coordinators link adults and children with chronic behavioral health and medical conditions to the services they need to stay as healthy as possible. Working in a team setting and primarily in the field, Care Coordinators assess risk and needs, develop person‑centered care plans, provide care management services, track and arrange appointments, educate members, and coordinate other aspects of members’ health and community services.

Responsibilities

Integrate medical, specialized, and behavioral health services with social and educational support services.

Periodically assess a member’s medical and behavioral health needs and compliance with recommended treatments.

Collaboratively develop an Individualized Care Plan (ICP) with the member, the member’s family or caregivers, and other service providers.

Track all specialty medical, behavioral, and support service referrals using the Health Information Technology (HIT) system.

Ensure members have access to, engage in, and retain needed services as defined in the ICP, including acute and primary medical care, preventive medical services, home health care, chemical dependency services, behavioral health services, community social support services, housing, state and federal entitlements, educational services, and involvement with child welfare, juvenile justice or criminal justice institutions.

Provide outreach services to increase member access to the above services.

Respond to member information and referral questions and reassess the need for ongoing care coordination services.

Complete all required documentation and share knowledge and experience with team members.

Maintain an agency‑provided cell phone and respond to member crises during (and occasionally outside of) regular business hours.

Perform other duties as assigned.

Educational / Training Requirements

Bachelor’s degree with a major or concentration (minimum 24 credits) in social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing; or NYS teacher’s certification; or NYS licensure and registration as a Registered Nurse plus two years of direct service or substantial case‑management experience; or a bachelor’s degree plus five years of relevant experience; or an associate degree or high school diploma/GED plus five years of experience.

Specific experience with the target population may be required.

Experience / Language Preference

Experience in interdisciplinary teams, care management or coordination in medical or behavioral health settings, and with chronically ill populations.

Fluency in a second language such as Spanish, Russian, or Creole is preferred.

Computer Skills

Proficiency with an Electronic Health Record (EHR) system.

Proficiency with Outlook and Microsoft Office applications.

Manual Dexterity

Ability to read paper and electronic documents and perform significant data entry.

Manual dexterity and hand‑eye coordination required for data entry and record keeping.

Work Environment / Physical Effort The role involves 20–40% office interaction and 60–80% field work (home visits, hospitals, clinics, benefit offices). Travel within New York City is required, carrying essential equipment up to 10 pounds. The job requires routine sitting, standing, and traveling using public or private transportation.
