About the Position Ensures continuous active treatment is provided to all persons in the caseload, according to their treatment programs and schedules, on a daily basis as set forth in ICF/IID regulations; serves as the team leader of the interdisciplinary team and leads monthly habilitation meetings; leads monthly Treatment Review meetings where each person served in the caseload has their total plan of care discussed covering progress towards successful completion of objectives, monitor for regression of skills already gained, and make sure changes are made to programs due to no progress or completion of criteria per monthly progress notes; complete a summary each month of all information discussed for each person served in Treatment Review; review, approve, and monitor their programs designed to manage inappropriate behavior; ensure person served are free from unnecessary drugs and physical restraints; monitor, maintain in good repair, and teach individuals to use/make informed choices about the use of eyeglasses, dentures, etc.; check programmatic notebooks weekly to ensure documentation sheets and information is in place and ensure documentation is being completed; check mechanical release/restraint forms, bedrest checks, and turn schedules for correctness on a daily basis; review all paperwork for correctness associated with monthly staffing for persons served in caseload including reviewing CFAs, evaluations, written training programs, 24-hour schedules, group schedules, etc., for correctness, continuity, and appropriateness; works closely with Support Care Professionals and will in-service staff on persons served as needs arise; Vacancy is at PDC Unit.
