Summary

This full‑time, day shift position will be a float position for the four facilities within the Spaulding network: Charlestown, Cambridge, Brighton, and the Cape (SRH, SHC, SRB, SCC).

Key Responsibilities

Complete psychosocial assessments covering psychological, social and environmental concerns, including evaluation of mental status. Review daily referrals and screenings of clients. Audit case files on a regular basis for completeness and accuracy. Assist in discharge planning as required. Serve and protect the hospital community by adhering to professional standards, hospital care policies and procedures, federal, state, and local requirements, and JCAHO standards. Perform other duties as assigned. Comply with all policies and standards.

Education

Master's Degree in Social Work (required). Master's Degree in related field of study (preferred). Master's Degree in Mental Health (required).

Licensing and Credentials

Licensed Clinical Social Worker (LCSW – State License) (preferred). Licensed Independent Clinical Social Worker (LICSW – State License) (preferred). Occupational Therapist (State License) (preferred). Physical Therapist (State License) (preferred). Certified Rehabilitation Counselor (CRC) (preferred). Licensed Mental Health Counselor (State License) (preferred). Licensed Rehabilitation Counselor (Massachusetts) (preferred). Respiratory Therapist (Massachusetts) (preferred).

Experience

Case management, utilization review, or discharge planning experience 2–3 years (preferred).

Knowledge, Skills & Abilities

Giving full attention to what other people are saying, taking time to understand the points, asking questions as appropriate, and not interrupting inappropriately. Using logic and reasoning to identify strengths and weaknesses of alternative solutions, conclusions, or approaches to problems. Establishing strong rapport and relationships with patients and staff. Proficiency in Microsoft Office and industry‑related software programs. Identifying complex problems and reviewing related information to develop and evaluate options and implement solutions. Maintaining client and staff confidentiality. Understanding of diagnostic criteria for dual conditions and ability to conceptualize modalities and placement criteria within the continuum of care. Knowledge of Healthcare and Managed Care (preferred).
