Magenta Care is a reputable home health agency dedicated to providing quality care to individuals in the comfort of their own homes. We are currently seeking skilled and compassionate physical therapists to join our team. The primary location is Seattle, Washington, with a requirement to travel up to one to two hours outside Seattle to serve clients.

Description Magenta Care is seeking a compassionate, skilled, and licensed In-Home Physical Therapist to join our mobile care team. In this role, you will deliver intermittent, one-on-one physical therapy services in the comfort of patients' homes. You will coordinate care plans, monitor progress, and provide essential education to patients and caregivers to support recovery and optimize function and mobility. You will collaborate closely with physicians, nurses, case managers, and other healthcare providers to ensure that each patient receives individualized and evidence-based care.

Responsibilities

Provide, coordinate, and oversee skilled intermittent physical therapy in the home setting.

Perform initial and ongoing assessments to determine patient needs and rehabilitation goals.

Educate patients, families, and caregivers on therapeutic exercises, safety precautions, and functional maintenance strategies.

Update and revise the plan of care (POC) in response to changes in patient condition, physician orders, or treatment goals.

Document patient visits, progress, and outcomes in a timely and compliant manner using our electronic health record system.

Communicate proactively with the patient's physician and case manager regarding clinical findings and recommendations.

Conduct follow-up evaluations within 48 hours after significant health changes or hospital discharges (of 24+ hours), or as ordered by the physician.

Maintain and protect patient confidentiality in accordance with HIPAA and company standards.

Basic Qualifications

Active Physical Therapist license in the State of Washington (required).

Minimum of 1 year of recent home health physical therapy experience (preferred).

Doctorate or Master's degree in Physical Therapy (Bachelor's degree acceptable with strong experience).

BLS Certification (Basic Life Support) from an approved provider.

Valid Washington State Driver's License and proof of auto insurance.

Reliable transportation for in-home visits.

Strong interpersonal skills and a patient-first mindset.

Comfortable using mobile devices and electronic documentation systems.
