About The Position The Senior Clinical Outreach Case Manager will help provide clinical oversight to the outreach team, conduct outreach to a caseload of high-priority unsheltered individuals experiencing homelessness, administer VI-SPDAT assessments, assist individuals in collecting identification documents to become document ready, and facilitate the housing process by acting as a liaison between the individuals and housing providers in various settings.

Duties and Responsibilities

Provide housing match services to facilitate successful housing placement.

Conduct outreach, case management, and housing navigation in settings such as streets, shelters, hospitals, jails, and encampments.

Offer light-touch and intensive outreach services as appropriate.

Refer clients to medical, mental health, and substance abuse services as needed.

Provide clinical oversight to outreach efforts, including trainings and case conferencing for complex cases.

Assist clients with applying for identification, entitlements, and benefits.

Collaborate with external providers like the Department of Behavioral Health.

Build rapport and utilize motivational interviewing to support clients through stages of change and acceptance of housing.

Conduct staff trainings.

Input assessments and upload documents into HMIS.

Maintain records and documentation, ensuring confidentiality.

Complete administrative tasks and attend required trainings.

Perform other duties as assigned.

Requirements Qualifications, Knowledge, Skills, and Abilities

Knowledge of the homeless services and housing landscape in Washington, DC.

Experience connecting with individuals experiencing mental health issues and/or substance abuse.

Strong interest and commitment to ending homelessness in DC.

Excellent communication skills and professional boundaries.

Understanding of systemic causes of homelessness.

Flexibility and comfort working in a fast-paced, diverse environment.

Ability to lift up to 20 lbs and handle physically demanding work, including standing and walking in all weather conditions.

Proficiency with computers, databases, and documentation.

Ability to navigate DC via public transportation and on foot.

Willingness to conduct outreach in various settings and weather conditions.

Proficiency in Spanish preferred.

Experience Minimum of 2 years in services for homelessness, mental health, substance abuse, HIV/AIDS, or case management for vulnerable populations.

Education

Master's degree in a clinical field.

At least 2 years of experience in social services.

Required: Valid license in social work, counseling, or related field.

Physical and Mental Demands Physical: Ability to sit, stand, walk, and lift objects up to 25 lbs. frequently in all weather conditions. Mental: Effective communication in English required.

Travel Demands Occasional local travel between Miriam's Kitchen and partner sites, with appropriate compensation for business travel.
