Early Intervention Services Coordinator (Case Management) for the Early Start Riverside North Unit

Description Under the supervision of the Program Manager - Early Start Program, the Early Intervention Services Coordinator is the first agency representative to families who have infants eligible for services under the U.S. Department of Education’s Individuals with Disabilities Education Act (IDEA), Part C. The Coordinator is responsible for coordinating assessment of these infants, educating parents regarding developmental strategies, arranging for appropriate service intervention, making appropriate case dispositions, and coordinating liaison activities with referring professionals. They develop and implement Individual Family Service Plans (IFSP), perform case management activities and community liaison tasks related to these cases, complete the intake process, and maintain a safe, effective service delivery. No supervisory responsibilities.
Hourly Range $25.67 – $39.82 per hour
Sign‑On Bonus
$250 sign‑on bonus after 90 days of employment with good standing.
$500 sign‑on bonus after 6 months of employment with good standing.

Essential Duties And Responsibilities
Daily case management responsibilities guided by the agency’s mission and core values.
Understand California’s regulations for the Early Start Program, the Lanterman Act, and differentiate eligibility and service provisions.
Maintain contact with consumers and families, ensuring rights and dignity are upheld.
Coordinate annual review process, including Baby IFSP, CDER, and Annual Review.
Provide advocacy services, crisis intervention, documentation, information and referral services.
Perform outreach community liaison duties as assigned.
Arrange placement of consumers in infant educational and other training programs approved by the Interdisciplinary Team.
Conduct assessments and developmental testing: First Look, Denver II, REEL III, Music 2 My Ears, M‑Chat, and Day‑C.
Instruct parents in developmental strategies and provide guidance on child care, illnesses, nutrition, safety, immunizations, and medical follow‑up.
Stay current on infant development, parenting skills, behavioral and medical information, community resources, and disability‑specific information.
Report caseload status to the Early Start Program Manager.
Represent the regional centre at case conferences and interagency meetings.
Complete all performance contract assignments.
Perform daily administrative tasks: purchase of service forms, record documentation, eligibility reviews, and quality alerts.
Complete at least 95 % of required paperwork within time frames and transition children reaching age three to appropriate education programs.
Prepare for compliance and administrative case reviews.
Accurately document TCM (Title 19) and Medicaid Waiver information.
Provide full case‑management coverage when requested.
Maintain good attendance, punctuality, and compliance with personnel and safety policies.
Contribute to team harmony and share expertise.
Adapt to changes and new priorities.
Use equipment accurately, maintain a neat work area, and comply with safety rules.
Organize travel efficiently and maintain a safe driving record.
Utilize bilingual skills as required.
Perform additional duties as assigned.
Minimum Position Requirements
Master’s degree in social work, psychology, public health nursing, teaching, special education, early childhood development, or a related field from an accredited institution.
BA or BS degree and one year of experience in case management within intellectual/developmental disabilities, social work, special education, early childhood development, or a related field.
Computer skills in Microsoft Office.
High adaptability and ability to handle change.
Strict confidentiality and reliable communication skills.
Strong verbal, written, and relationship‑building skills.
Valid driver’s license, automobile liability insurance, and a safe driving record.
May require bilingual capability.
Option For candidates who meet the educational requirement but not the experience requirement, we are accepting applications for a
Case Management Trainee
(CMT). The CMT is a year‑long trainee position with a minimum requirement of a Bachelor’s degree from an accredited college or university.
