Under the supervision of the Program Manager – Early Start Program, the Early Intervention Services Coordinator is the first agency representative to families that have infants eligible for services under the U.S. Department of Education’s Individuals with Disabilities Education Act (IDEA), Part C. The coordinator is responsible for coordinating assessment of these infants, educating parents regarding developmental strategies, arranging appropriate service intervention, making appropriate case dispositions, and liaison activities with referring professionals. They develop and implement the Individual Family Service Plan (IFSP), perform case‑management activities, and community liaison tasks related to these cases. The position involves completing the intake process and has no supervisory responsibility.
This position provides services throughout the San Bernardino Desert and Mountains area, including travel to many remote communities. Mileage reimbursement for business travel is paid on a monthly basis.
Job Details Hourly Rate : $25.67 – $39.82.
Sign‑on bonus : $250 after 90 days and $500 after 6 months of employment.

Essential Duties and Responsibilities
Assume daily case‑management responsibilities in line with the agency’s mission and core values.
Understand California’s regulations for the Early Start Program, Lanterman Act, and differentiate eligibility and service provisions for each program.
Maintain contact with consumers and families as required, ensuring their rights and dignity.
Coordinate the annual review process, including completion of Baby CDER, CDER, Annual Review, and IFSP.
Provide advocacy services and individual/family crisis intervention with proper documentation.
Perform outreach community liaison duties as assigned.
Arrange placement of consumers in infant educational or other training programs approved by the Interdisciplinary Team.
Perform assessments and developmental testing (First Look, Denver II, REEL III, Music 2 My Ears, M‑Chat, Day‑C).
Instruct parents in developmental strategies and provide anticipatory guidance on child care, health, nutrition, safety, and immunizations.
Keep current on infant development, parenting skills, behavioral information, community resources, and disability‑specific topics.
Inform the Early Start Program Manager of caseload status.
Represent the regional center at case conferences and interagency meetings.
Complete all assignments arising from the agency’s Performance Contract.
Submit administrative documentation (Purchase of Service forms, consumer file records, eligibility reviews, SIR closures, quality alerts) within required timelines.
Transition children reaching age three to appropriate education programs, initiating the process early to avoid delays.
Prepare for compliance reviews, eligibility reviews, and other case reviews.
Complete TCM (Title 19) and MediCaid Waiver documentation accurately and promptly.
Maintain attendance, punctuality, and comply with Personnel Policies, Injury and Illness Prevention Plan, and reporting of unsafe conditions.
Collaborate effectively as a team member, sharing expertise and contributing to team harmony.
Demonstrate flexibility and adaptability in managing interruptions and new priorities.
Use agency equipment responsibly, keep work area tidy and safe, follow IT security protocols, and maintain a safe driving record.
Utilize bilingual skills as needed.
Perform additional duties as assigned.
Minimum Position Requirements
Master’s degree in social work, psychology, public health nursing, teaching, special education, early childhood development, or a related accredited field.
Or a BA/BS degree with one year of relevant case‑management experience.
Computer proficiency in Microsoft Office.
Ability to handle change, maintain confidentiality, follow directions, and establish effective relationships.
Good verbal and written communication skills.
Valid driver’s license, automobile liability insurance, safe driving record, or ability to provide alternate transportation.
May be required to be bilingual.
Case Management Trainee Option For candidates meeting the education requirement but lacking experience, a one‑year
