We serve nearly 80,000 individuals and families, including almost 13,000 youth, from communities across Brooklyn, the Rockaways, and beyond. Office of Mental Health (OHM) Supported Housing Program
OHM provides 87 scattered‑site units of housing to individuals eligible for the program. The first program offers 70 units to chronically homeless single adults with SSI/SSD and serious persistent mental illness (SPMI) or who are diagnosed as Mentally Ill and Chemically Addicted (MICA). The second program supplies 10 units to SPMI individuals residing in OMH psychiatric centers, Article 28 inpatient hospitals, residential treatment programs, and adult homes. The third program provides 7 units to SPMI individuals who have resided in OMH psychiatric centers or OMH‑operated residential programs for one year or longer. All apartments are located in Brooklyn. The case‑management services focus on removing barriers to maintain housing by ensuring clients’ basic needs are met and connecting them to needed services, such as mental health and substance abuse. The operations component ensures housing units meet habitability standards through repairs, new unit identification, lease negotiation, and vacancy readiness. Clients contribute 30% of income toward rent. Position: Case Manager

The Case Manager provides all contract‑funded activities, including conducting Intakes, Assessments, Service Plans, Reassessments, Service Plan Updates, Accompaniment, Referrals, Advocacy, Housing Inspections, Case Conference, and related program documentation. The Case Manager enters all client services into the Client Tracking System and ensures clients engage in primary medical care and adhere to treatment protocols. Connect clients to needed services – medical, mental health, substance use treatment, legal, entitlements, and more. Monitor clients’ progress toward goals via scheduled telephone contact and/or face‑to‑face home and office visits, documenting progress notes. Act as client liaison and advocate with outside organizations on education, healthcare, housing, and legal matters. Escort clients to appointments, assist with benefits applications, and monitor application outcomes. Collect required documentation and create client files. Conduct a minimum one home visit per month. Report housing repair issues to the Housing Specialist. Monitor clients’ living conditions and meet required monthly projections. Provide required information for monthly program reports and maintain audit‑ready client charts. Perform other duties as assigned. Minimum Education/Experience Required
Bachelor or Associate degree with one year of equivalent experience, or Two years of equivalent experience with sufficient education and technical expertise, including written and oral instruction comprehension, basic math, and task documentation. Relevant experience, preferably in a mid‑size organization (250+ employees) supporting people with histories of homelessness, mental illness, and/or substance abuse. Good written and verbal communication skills. Computer literacy in Microsoft Office Suite. Bilingual English and Spanish (preferred). Other Requirements
