Case Manager, Justice Involved Supported Housing Initiative (JISH)
We are seeking a Case Manager to support clients in the Justice Involved Supported Housing Initiative (JISH) at CAMBA, a community organization dedicated to providing comprehensive services across New York City.

19 Winthrop Street, Brooklyn, NY 11225
Reports To:
Program Manager
Position Type:
Full-time (35 hours per week)
Compensation:
$51,500 annually
What the Case Manager Does
Conducts and completes intakes, assessments, service plans, reassessments, and updates; documents services in Client Track; ensures clients maintain primary medical care and advance medication treatment compliance.
Connects clients to medical, mental health, substance use treatment, legal services, entitlements, and other needed supports.
Monitors client progress toward goals through scheduled calls, face‑to‑face visits, and progress notes.
Acts as client liaison and advocate with outside organizations on education, healthcare, housing, and legal matters.
Exits clients to appointments and assists with benefit and entitlement applications.
Collects documentation, creates client files, conducts at least one home visit per month, reports repair issues, monitors living conditions.
Meets monthly projection requirements, supplies data for program reports, and maintains audit‑ready charts.
Performs additional duties as assigned.
Minimum Education and Experience
Bachelor’s or Associate’s degree with one year of equivalent experience, or
Two years of equivalent experience with sufficient education and technical expertise for written and oral instructions, basic math, and documenting tasks.
Relevant experience, preferably in a mid‑sized (250+ employees) organization working with persons with histories of homelessness, mental illness, and/or substance abuse.
Good written and verbal communication skills.
Computer literacy in Microsoft Office Suite.
Bi‑lingual English and Spanish preferred.
Other Requirements
Disclosure of any subject to a State Central Child Abuse Registry report.
