FTE:

Full Time, 1.0 FTE (Day – 08 Hour).

Location:

Palo Alto, CA, 1820 Embarcadero.

Purpose:

To coordinate continuum of care and discharge planning for assigned patients, ensuring timely, efficient use of resources and compliance with external review agencies.

Responsibilities

Coordinate patient transition through the system; transfer accountability to appropriate care teams upon entry or discharge.

Facilitate timely implementation of discharge plans for patients with complex needs; arrange follow‑up care.

Conduct utilization review of all inpatients; monitor length of stay; collaborate with physicians to ensure resource utilization aligns with covered benefits and patient needs.

Provide education and consultation to patients, families, and hospital personnel regarding the discharge planning process and regulatory requirements.

Lead administrative/clinical support staff assisting with case management of an assigned caseload.

Participate in department program planning, goal setting, system development, and process improvement; develop documentation of findings and discharge arrangements.

Collaborate with interdisciplinary teams to assess, plan, and coordinate patient care needs; develop plan of care.

Identify and participate in quality improvement activities, focusing on clinical outcomes and resource utilization.

Collect, analyze, and report utilization and quality data; communicate with third‑party payers to assure reimbursement and compliance.

Education and Experience

Minimum

Bachelor’s Degree

from an accredited institution.

Master’s Degree preferred.

3+ years of progressively responsible and directly related nursing experience.

Active

Registered Nurse (RN)

license in California or equivalent; State or Compact Licensure required.

Knowledge, Skills, and Abilities

Data collection, evaluation, and reporting; proficiency in statistical analysis.

Patient assessment, care planning, and coordination across settings.

Effective communication with patients, families, clinical teams, and payers.

Conflict resolution, negotiation, and interpersonal relationship building.

Understanding of federal and state compliance laws, hospital operations, and financial processes.

Knowledge of case management models, utilization review, discharge planning, and quality improvement.

Compensation Base pay scale: $74.73 – $99.04 per hour, determined by internal equity, education, experience, and other factors.
