Social Worker (MSW) – Veteran Shelter Program

📍 Location: Northport, NY

Pay Rate: $30/hr

🕒 Full-Time | Flexible Schedule (Standard M–F, 9 am–5 pm)

Job Summary

We are seeking an MSW Social Worker to provide direct and indirect services to Veterans in a shelter setting. The Social Worker will help clients improve quality of life, secure permanent housing, and achieve long-term stability through individualized plans, resource coordination, and ongoing support. The facility operates 24/7, offering flexible scheduling around standard weekday hours.

Responsibilities

• Complete intake assessments, respond to critical needs, and request DD214 records.
• Orient clients to program expectations, confidentiality, and services.
• Ensure clients’ hygiene, daily living, and nutritional needs are supported.
• Develop and update Independent Living Plans (ILPs); track progress weekly.
• Make referrals to housing, mental health, medical, and community services.
• Maintain accurate case records, progress notes, and HMIS entries.
• Report critical incidents and provide updates to the Program Director.
• Participate in fire/emergency drills and follow safety procedures.
• Coordinate with maintenance for repairs and with VA liaison for client goals.
• Participate in rotating on-call weekend coverage and weekday coverage when needed.
• Maintain professional conduct, positive interactions, and confidentiality.

Requirements

• MSW required (LMSW/LMHC preferred)
• Case management and HMIS experience preferred
• Experience in residential or homelessness programs preferred
• Knowledge of mental health, substance use, or co-occurring disorders preferred
• Strong communication, documentation, crisis-response, and time-management skills
