About the Role
As the
Lead Care Manager , you will guide members through every step of their care journey—addressing medical, behavioral, housing, and social needs with a whole-person approach. You’ll be the connector between members, providers, and community partners, ensuring individuals receive the support and resources needed to achieve stability and better health outcomes. This role blends
care coordination, leadership, advocacy, and real community impact —making it an excellent next step for experienced case managers wanting to grow.
✨ What You’ll Do
Build trust through frequent in-person visits in homes, shelters, and community settings. Complete assessments and develop individualized care plans tailored to each member’s needs. Coordinate medical, behavioral health, housing, transportation, and social services. Advocate for members by navigating insurance authorizations, appeals, and complex barriers to care. Serve as the central communication hub between members, families, providers, and community organizations. Connect members to critical resources: food support, housing programs, job training, home health, and more. Ensure compliance with Medi-Cal, CalAIM, and ECM program standards. Provide leadership and support to team members, contributing to improved workflows and care quality.
What We’re Looking For
3–5 years of experience in case management, social services, or healthcare settings Strong understanding of
Medi-Cal, CalAIM, and ECM
programs Knowledge of Monterey County community resources and healthcare systems Skilled in care planning, motivational interviewing, and service coordination Organized, proactive, and able to manage multiple priorities Comfortable with technology and case management documentation systems
