📍 Location: Washington, DC 🕒 Shift: 8:30 AM – 5:00 PM, Monday – Friday 💵 Pay Range: $55 – $60 per hour

Position Overview

We are seeking a compassionate and experienced Clinical Social Worker (LCSW) to join our hospital-based care team in Washington, DC. The ideal candidate will provide comprehensive clinical and case management services, including discharge planning, patient advocacy, and psychosocial support for patients and their families.

Key Responsibilities

• Conduct psychosocial assessments and develop individualized discharge plans.
• Coordinate patient care and collaborate with the interdisciplinary healthcare team.
• Provide counseling and crisis intervention for patients and families.
• Facilitate access to community resources and post-discharge support.
• Document assessments, plans, and interventions accurately in Cerner.

Qualifications

• Education: Master’s Degree in Social Work (MSW) required.
• Licensure: Active DC Social Work License (LGSW, LICSW, or LCSW).
• Experience: Minimum 1 year of hospital case management and discharge planning experience required.
• Technical Skills: Working knowledge of Cerner electronic health record system preferred.

Schedule & Compensation

Shift: Monday through Friday, 8:30 AM – 5:00 PM

Pay Rate: $55 – $60 per hour (based on experience and qualifications)
