Nurse Practitioner/ Physician Assistant – In Home Assessments – San Jose, CA
Base pay range: $113,332.00/yr – $169,999.00/yr
Conduct in‑home assessments 3 days a week (with some outpatient clinical work) and work from home 2 days a week. Explore and manage complex and chronic care needs of senior patients through interdisciplinary teamwork.
Responsibilities
Conduct in‑home assessments, including comprehensive history, physical exam, medication review, and appropriate cognitive/depression/safety screenings.
Perform laboratory specimen collection in the home setting when appropriate.
Identify diagnoses for active medical and care management of patients, focusing on chronic disease management.
Communicate assessment findings to primary care providers to address care gaps.
Provide patient, family, and caretaker education, emphasizing close monitoring and follow‑up of patient needs.
Maintain knowledge of community resources for referral.
Comply with HIPAA regulations and protect PHI.
Engage in end‑of‑life care discussions as needed.
Job Requirements
Experience:
One (1) year of prior clinical or home care experience preferred; prior EMR and geriatric care experience also preferred.
Education:
Master’s degree from an accredited NP or PA program.
Licensure:
Active California state Nurse Practitioner or Physician Assistant license; board certification. NPI Number, DEA, Valid BLS, valid California driver license, and current automobile insurance.
Specialized Skills:
Effective communication with patients and team members. Problem‑solving, organizational, and time‑management skills in a fast‑paced environment.
Physical Requirements:
Regular standing, walking, reaching, lifting and moving up to 20 pounds; clear vision is required.
Benefits
