Summary: Seeking experienced Physical Therapist candidates with Home Health experience in several locations across Indiana and Michigan:
Fort Worth, IN Indianapolis, IN Escanaba, MI Farmington Hills, MI Marquette, MI
The Home Health Physical Therapist (PT, DPT, RPT) evaluates patients’ functional status, mobility, endurance, pain, and related medical conditions to determine therapy needs. Based on these findings, the PT develops and implements a treatment plan, provides interventions to improve function and well-being, and monitors progress.
Key responsibilities include: Assessing physical therapy needs and assisting the physician in creating the plan of care. Monitoring patient response to treatment and reporting changes to the supervising nurse or physician. Delivering therapy to reduce pain, restore or maintain function, and promote optimal independence. Educating patients, families, and care team members on the care plan, limitations, and progress.
Licensure Requirements
Current Physical Therapy licensure in state of practice. Current CPR certification required. Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
