Your Role:
Deliver personalized, high‑quality therapy services to residents in our skilled nursing facility
Evaluate, develop, and implement individualized treatment plans
Provide ongoing physical or occupational therapy services and reassessments
Document therapy services accurately and in compliance with all regulations
Work closely with the interdisciplinary team to ensure the best outcomes for residents
Promote independence, mobility, and quality of life for every resident you serve
Follow all facility policies, procedures, and professional standards
Who We’re Looking For:
Current
Physical Therapist (PT)
or
Occupational Therapist (OT)
license in Colorado or a valid compact license
Passionate about improving residents’ quality of life through therapy
Strong communication and documentation skills
Experience in a skilled nursing or long‑term care setting is a plus, but not required
Collaborative team player committed to excellent care
