Rochester Regional Health is seeking an experienced physician leader to serve as
Medical Director for ElderONE , the region's Program of All-Inclusive Care for the Elderly (PACE). ElderONE provides comprehensive, coordinated care that helps frail older adults safely remain in their homes and communities.
Key Responsibilities
Provide clinical leadership for all ElderONE/PACE sites
Lead and mentor interdisciplinary care teams
Ensure compliance with CMS and NYSDOH PACE regulations
Oversee care planning, quality initiatives, and clinical standards
Contribute to strategic planning, program operations, and performance improvement
Qualifications
M.D. or D.O., board certified in Geriatrics, Internal Medicine, or Family Medicine (Geriatrics preferred)
Eligible for NY medical licensure
Minimum 5 years of relevant clinical experience; leadership experience strongly preferred
Strong communication, collaboration, and team leadership skills
About ElderONE / PACE ElderONE, sponsored by Rochester Regional Health, is the oldest PACE program in New York State. PACE provides comprehensive medical and social services to eligible older adults through an interdisciplinary team approach, enabling participants to remain safely at home rather than entering a nursing facility. Services include primary and specialty care, rehabilitation, transportation, social engagement, and 24/7 clinical support.

PHYSICAL REQUIREMENTS L - Light Work - Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly; requires occasional walking, standing or squatting.
PAY RANGE $250,000.00 - $300,000.00
The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, clinical licensure date, relevant qualifications, specialty, internal equity, location, and contracts.
