Overview
Post Acute Network, Care Manager
Full-Time, 40 hours per week. Monday through Friday, 8:00 a.m. to 5:00 p.m.; flexibility to work later as needed

The Post Acute Network Care Manager role works in collaboration with skilled nursing facilities to promote quality outcomes for patients with complex medical needs. Responsible for increasing efficient utilization of time at the healthcare facility; identifying barriers and ensuring the appropriate level‑of‑care is provided. Evaluates and ensures that the patients' medical, environmental and psychosocial needs are met while at the facility and over the continuum of care. The Post Acute Network Care Manager acts as an advocate through coordination and communication with the facility health care team to implement recovery goals and prevent hospitalization re‑admissions.
Responsibilities
Manage a weekly case load of patients with complex medical needs, across multiple skilled facility networks
Review, identify, and work to remove patient barriers to discharge through collaborative facility discussions and resources
Identifies patients prior to discharge for additional Duly services, and coordinates as needed, preparing patients for a successful discharge
Communicates and escalates patient or facility issues as they arise
Facilitate patient handoff report to Duly CM for continued support when discharged from the skilled facility
Coordinate Health Care Services with appropriate physicians and health care providers if applicable
Manage and provide a comprehensive summary and log of all active patients
Perform additional duties/tasks as assigned
Licensure/Certification
Must be an active Registered Nurse License in the State of Illinois in good standing.
Certification in Case Management, preferred.
Current CPR certification must be maintained (BLS).
Education
Associate’s degree in nursing required; Bachelor’s degree in nursing preferred.
Experience
2-4 years of experience working as a Registered Nurse with the adult/older adult patient population in the primary care or hospital setting. Skilled nursing facility, emergency department, urgent care, adult medical/surgical, or ICU experience strongly preferred.
Case management or utilization management experience preferred.
Ability to communicate effectively with physicians and nurses.
Experience within a multi‑tasking environment required.
The compensation for this role includes a base pay range of $67k-101k, with the actual pay determined by factors such as skills, experience, education, certifications, geographic location, and internal equity. Additional compensation may be available through shift differentials, bonuses, and other incentives. Base pay is only a portion of the total rewards package.
