Responsibilities

Serve as liaison to guardians, family members, and legally authorized representatives (LARs) for routine and non‑routine notifications (e.g., abuse and neglect allegations, consent for treatment, special events).

Gather, review, and document case information for new admissions and other assignments; prepare social histories and contact information sheets.

Collaborate with the transition specialist to coordinate community referrals, including attending referral meetings, site visits to group homes and vocational services, and following up on identified issues.

Assist the assigned social worker and/or admissions staff with comprehensive reviews of case information and provide necessary support.

Design and conduct individual and group training on assigned topics (e.g., rights, decision‑making, self‑advocacy skills) in accordance with licensure requirements.

Monitor compliance with policies related to human rights, family contact, admissions, and placement; perform administrative investigations and root‑cause analysis of incidents.

Assist in completing rights and decision‑making assessments and evaluating requests related to residents’ trust funds.

Provide technical assistance and administrative support for admissions, transfers, transitions, discharges, and furloughs, ensuring access to SSLC services through person‑centered practices.

Plan, promote, and coordinate special events (e.g., family meals, provider fairs, holiday events) to increase awareness of the SSLC program.

Conduct surveys and reviews to ensure residents, LARs, and other stakeholders receive required services and improve overall service quality.

Participate in agency disaster response, continuity operations, and any other duties as assigned.

Knowledge, Skills & Abilities (KSAs)

Knowledge of Texas Administrative Code, HHSC policies, and procedures related to State Supported Living Centers.

Knowledge of client protections, human rights, and intermediate care facility regulations.

Understanding of marketing techniques and program performance indicators.

Computer skills: Microsoft Word, Outlook, PowerPoint, and Excel.

Facilitate meetings, group and individual training sessions.

Gather, assemble, correlate, and analyze facts; collaborate to devise solutions to administrative and technical problems.

Communicate effectively, orally and in writing, with staff, IDT, residents, and family members/LARs.

Analyze root‑cause problems and develop plans of correction; prepare reports and conduct surveys.

Initial Screening Criteria

Bachelor’s degree from an accredited college or university and one (1) year of experience performing social services work (case management, individual or group training/education, case reviews).

OR High school diploma or equivalent and five (5) years of experience performing social services work (case management, training/education, case reviews).

One (1) year of experience working with people with intellectual and developmental disabilities is preferred.

Additional Information

Applicants must pass a fingerprint criminal background check, pre‑employment drug screen, and registry checks (CANRS, Nurse Aide, Medication Aide, Employee Misconduct, LEIE).

All State Supported Living Center employees are subject to random drug testing.

Flexibility in work hours may be required; the position may require overtime, extended hours, weekends, and holidays.

Males between the ages of 18 – 25 must be registered with the Selective Service.

All applicants must be at least 18 years of age to be considered for employment at a state‑operated facility.

Applicants must bring I‑9 documentation on the first day of work; HHSC uses E‑Verify.
