Are you a compassionate and experienced professional with a background in nursing, social work, occupational therapy, physical therapy or a related field? Do you have a passion for working with older adults and their families? If so, we want you to join our growing multidisciplinary team!

As a Care Manager at LiveWell Eldercare Management, you will have the opportunity to:

Conduct in-home assessments and develop care plans that meet the unique needs of each client Build strong relationships with clients, families, and care providers Advocate for clients and help them navigate the complex healthcare system Provide support and education to caregivers Make a real difference in the lives of older adults and their families

We are looking for someone who is:

Highly organized and self-directed An excellent communicator with outstanding interpersonal skills Compassionate and patient Proficient in using technology A team player with a positive attitude

Qualifications:

Advanced degree preferred and licensure required in social work, nursing, geriatrics, mental health, occupational therapy, physical therapy, or related field 3-5 years of experience working with older and disabled adults and families Excellent clinical skills; knowledge of family dynamics, medical system, health insurance, community resources Highly organized, self-directed, ability to work in non-traditional, professional work environments, and manage fee-for-service caseload effectively Outstanding interpersonal and communication skills; compassionate Knowledge and comfort using a laptop/desktop computer and smartphone Ability to drive, access safe and reliable vehicle Obtain and/or maintain membership in the Aging Life Care Association (ALCA) Maintain professional licensure as required by the Commonwealth of Massachusetts

Duties:

Conduct professional, objective in-home assessments of older and disabled adult clients and their families; communicate with caregivers and medical providers as needed. Develop professional, effective relationships with clients, families, responsible parties, and care providers in all care settings. Create and effectively communicate client-centered, professional, objective, creative recommendations for care, resources, education, and support to clients, families Assist in the implementation and oversight of recommendations Provide client-centered problem-solving, ongoing support, and advocacy for clients as needed Arrange for and/or accompany clients to medical appointments, provide feedback to family, and ensure that medical orders are followed through, as needed. Provide support and education to family/loved ones coping with caregiver stress, loss, dementia, illness, change, and resistance. Document billing and interventions daily Attend biweekly team meetings and one-to-one meetings. Abide by Standards of Practice for Aging Life Care Professionals as determined by ALCA
