Lead Navigator Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of programs throughout Arizona. CBI provides residential, outpatient, inpatient, physical health, medication‑assisted treatment, and crisis services to individuals experiencing substance use disorder, homelessness, and mental illness.

Job Summary A Lead Navigator is a professional in recovery from AOD use for ≥1 year with at least 90 days experience as a Navigator or Peer Support Specialist. The Lead Navigator works with individuals who are homeless, providing outreach and supportive services to assist them with ending homelessness. The Lead Navigator coordinates and monitors community‑based services, facilitates service delivery, and mentors Navigator II staff.

Responsibilities

Conduct outreach, engagement, case management, and ongoing wrap‑around supportive services.

Coordinate care with community providers and stakeholders.

Explain and implement coordinated entry, housing referral, leasing up, wrap‑around services, and discharge processes.

Collaborate as part of a multidisciplinary team including nurses, counselors, and clinical leads.

Serve as a bridge for the patient, ensuring successful transition to and from community services.

Qualifications

High school diploma or GED required; Associate degree in behavioral health preferred.

1–3 years of full‑time health‑care related experience with exposure to behavioral and medical crisis situations.

Current Arizona driver’s license and clear 39‑month motor‑vehicle record.

Behavioral Health Technician credential in accordance with A.A.C. R9‑10‑101.33 and CBI clinical policies.

Arizona Fingerprint Clearance card.

Demonstrated recovery and peer support experience.

Skills & Requirements

Recovery from substance use and/or GMH for ≥1 year.

At least 90 days of Navigator or Peer Support Specialist experience.

Strong communication and case‑management skills.

Ability to work independently and mentor staff.

Knowledge of housing services and community resources.
