The Opportunity
We are forming a select Advisory Board of healthcare and innovation leaders to guide our next phase of growth. Advisors will provide high-level strategic insight, support partnership development, and contribute to shaping the future of care transitions across hospitals, SNFs, and value-based organizations.
Advisors will also receive early visibility into Talamel’s current Seed Round, with the option to participate alongside other healthcare executives and early supporters.
Key Responsibilities Clinical & Operational Guidance Provide insight into discharge planning, care transitions, case management workflows, or post-acute coordination. Technology & Workflow Integration Offer feedback on integration with hospital processes, SNF operations, and PCP communication pathways. Strategic Introductions Support introductions to health systems, SNFs, PCP networks, ACOs/IPAs, or payor groups. Adoption & Market Insight Share perspectives on system-level decision-making and care coordination needs.
Ideal Profiles
We are especially seeking individuals with:
10+ years in hospital operations, case management, post-acute care, or value-based care Leadership experience in health systems, SNFs, ACOs/IPAs, or managed care Expertise in care transitions, discharge workflows, or patient navigation Background in digital health, care coordination, or health IT Strong industry networks Interest in early-stage innovation and healthcare system transformation
Founding Team Vivek Bhatia — Founder & CEO 18+ years leading healthcare transformation across payer and provider organizations. Former Director at Blue Shield of California with a proven record of executing large-scale enterprise initiatives. Jukka Valkonen — Co-Founder, Head of Clinical Solutions RN/PHN with 35 years of clinical experience across hospitals and post-acute settings. Expert in discharge workflows and frontline operations. Jim Collette — Head of Technology Former Chief Architect at Jiseki Health. Specializes in scalable, secure, compliant healthcare infrastructure.
