Position Summary
Responsible for ensuring evidence based care is delivered to all patients. Responsibilities
Coordinates the daily operations of the specialized clinical program with a strong focus on quality assurance and improvement Responsible for working in collaboration with the healthcare team to coordinate the care and service to patients across the continuum of care Collaborates the plan of care utilizing standardized care protocols in accordance with nationally recognized evidence based care guidelines Responsible for managing the program including but not limited to community education, prevention, diagnosis, treatment, therapy, and regaining functional status Assists patients in understanding their diagnosis, treatment options and the resources available A wide degree of creativity and latitude is expected May lead and direct the work of others Performs other related duties as assigned Familiar with standard concepts, practices, and procedures within the field Relies on experience and judgment to plan and accomplish goals Regular and predictable attendance is an essential job function Competent to meet age specific needs of the unit assigned Requirements and Minimum Qualifications
BSN required State of Idaho Registered Nurse or licensure pending as documented by temporary licensure OR current RN licensure from a state participating in the multi-state privilege to practice compact with Idaho required Previous experience in program development Clinical knowledge base and data management skills Knowledge of regulatory standards, evidence-based guidelines Strong organizational and communication skills Working Conditions
