Brockton Home Health Care AFC is looking for a registered Nurse (RN)/Case Manager to manage and support our Adult Family/Foster to ensure the programs run smoothly.
About the Position We are seeking an
experienced Office Assistant
with direct knowledge of Massachusetts Adult Foster Care (AFC) regulations and hands-on experience using the
Axxess
portal.
Position Overview The Office Assistant will play a vital role in supporting daily operations, ensuring accurate documentation, and maintaining compliance with Massachusetts AFC guidelines. This role requires excellent organizational skills, strong attention to detail, and the ability to navigate and manage the Axxess platform efficiently.
Key Responsibilities
Assist with day-to-day administrative tasks to support AFC operations.
Enter, update, and manage client and caregiver information in the Axxess portal.
Ensure all documentation meets Massachusetts AFC compliance standards.
Fax MDS to PCPs office and track all referrals.
Handle phone calls, emails, and correspondence with caregivers, members, and external partners.
Prepare and maintain files, reports, and records.
Coordinate with the care team to track assessments, care plans, and visits.
Assist with billing and payroll processes as needed.
Qualifications
Required:
At least 1 year of Adult Foster Care Agency experience.
Required:
Proficiency in the Axxess portal.
Strong organizational skills and attention to detail.
Proficient in Microsoft Office (Word, Excel, Outlook).
Excellent verbal and written communication skills.
Ability to maintain confidentiality and work independently.

9:00 AM – 5:00 PM (with some flexibility)
Job Type: Part-time
Pay: $24.00 - $25.00 per hour
Expected hours: 20 – 25 per week
Work Location: In person
Additional Qualifications
Proven experience as an administrative assistant or office administrator.
Knowledge of office management systems and procedures.
Working knowledge of office equipment, like printers and fax machines.
Proficiency in MS Office (Excel and PowerPoint, in particular).
Excellent time management and ability to prioritize work.
Attention to detail and problem-solving skills.
Excellent written and verbal communication skills.
Strong organizational skills with ability to multi-task.
High school degree; additional qualification as an Administrative assistant or Secretary is a plus.
Bi-lingual preferred (Portuguese, Cape Verdean Creole, and Spanish).
