Assessment & Referral Registered Nurse
Recruiting for Sacramento Behavioral Healthcare Hospital (SRBHH), a North Bay acute psychiatric inpatient facility with 144 beds. The role focuses on accurate assessment of incoming admissions and delivering high‑quality, safe care during the intake process.
Salary : $63.00‑$78.00 per hour (≈$131,040‑$162,240 annually).
Reports to : Director of Assessment and Referral.
Hours : Part‑Time, Weekends.
Key Responsibilities
Respond to telephone and face‑to‑face inquiries from persons seeking mental health care or information about mental health‑related services.
Use a systematic process of interaction with patients, significant others, and healthcare providers to collect and analyze data regarding the patient’s physical, psychosocial, environmental, self‑care, educational, spiritual, cultural, and anticipated discharge needs.
Determine patient’s alcohol or drug toxicity; identify signs of withdrawal; conduct screenings or assessments to determine treatment needs.
Render care and treatment competently and timely, based on current chemical dependency and psychiatric nursing/clinical knowledge and standards; evaluate care against goals and revise as needed.
Facilitate the patient’s access to the appropriate level of care and setting.
Establish and maintain effective working relationships with patient services counselors, coordinators, intake department, physicians, allied health professionals, and nursing staff.
Assess crisis and clinical indicators via phone and in‑person and document such on a call sheet; schedule assessment as clinically appropriate.
Prepare written reports of assessment results promptly, including recommendations for the level of care and treatment needs.
Make appropriate interventions during assessments and crisis situations with clients, family/friends, and other professional staff.
Assess need for involuntary hospitalization; contact county crisis team, law enforcement, or 5150 Certified employees as needed.
Enter data of calls and intake assessments in the computer system timely.
Assist clients not admitted into a program with clinical referrals and recommendations.
Provide pertinent clinical information to physicians and program staff to ensure a smooth transition for the patient.
Distribute documentation of admissions to the Business Office, Admitting Unit, and Utilization Review Department.
Maintain knowledge of developmental tasks and sociological theories for pertinent age groups; keep updated with organizational programs and services.
Participate in activities that enhance professional growth and development; uphold the organization’s ethics and customer‑service standards.
Cross‑train as Inpatient Registered Nurse and perform duties such as direct patient care, documentation, medication administration, and team communication during shift hand‑offs.
Perform all duties assigned and uphold high standards of patient safety and quality of care.
Knowledge and Experience
Current Registered Nurse licensure in the state of California.
One year of nursing experience or more required; inpatient experience preferred.
Familiarity with continuum of care and clinical terminology.
Knowledge of Diagnostic and Statistical Manual of Mental Disorders (DSM) diagnostic criteria.
Current CPR certificate or to be obtained during initial orientation.
Familiarity with the verbal de‑escalation and physical intervention program approved by the California Nursing Council.
Skills and Abilities
Maintain patient confidentiality at all times.
Knowledge of patient rights and laws related to mental health.
Assessment and intervention skills, including crisis management.
Persuasive communication to overcome denial and resistance to treatment.
Capacity to concentrate in high‑traffic, challenging environments.

Coordinate multiple tasks simultaneously; reach, lift, and move objects up to 50 lbs.
Stand, sit, or walk for at least one hour at a time throughout the day.
Provide manual restraints for patients who may act out; maintain visual acuity standards.
Work predominantly indoors with minimal exposure to adverse environmental conditions.
