Job Summary The Home Health RN is responsible for delivering high-quality, patient-centered care in the home setting. Responsibilities include assessing patient conditions, developing and implementing individualized care plans, and providing ongoing education and support to patients and their families. Care is delivered in alignment with the organization’s philosophy, policies, and community health practice standards.

Responsibilities The Home Health RN also manages admission processes and fulfills case management duties in accordance with MGB Home Care policies and professional standards. This role requires thorough and timely electronic documentation, effective interdisciplinary communication, and a strong commitment to patient safety and quality outcomes.

Qualifications

Minimum 1 year of certified home care experience

Minimum 1 year of case management experience

Ability to travel to assigned patient cases

Weekend rotation required once every 6 weeks
