Job Summary
The Transitional Age Youth (TAY) Supportive Services Case Manager (Bilingual) reports to the Assistant Director of TAY Rapid Rehousing. This role provides housing‑focused case management services in English and/or Spanish to San Francisco clients (ages 18–27) who receive rapid rehousing rental assistance through the San Francisco Department of Homelessness and Supportive Housing and other city departments. The position requires a strong commitment to supporting youth as they transition out of homelessness, helping them build the skills and stability needed for long‑term self‑sufficiency. Team members demonstrate professionalism and integrity in every aspect of their work, ensuring the highest standards of service for clients, community partners, and colleagues. Job Duties and Responsibilities
Case Management Duties
Assist TAY participants with securing housing by providing navigation support, including viewing units, completing applications, and accessing credit and background reports. Support participants in understanding tenant rights, lease responsibilities, and requirements. Guide on timely rent payments and coach effective communication and conflict resolution with landlords. Develop case management plans and connect clients with community and government resources to prevent loss of benefits and increase housing stability. Deliver supportive services to stabilize housing, enhance well‑being, and build self‑sufficiency. Make referrals and coordinate services that lead to increased housing stability, income, job placement assistance, and retention. Work with the rental assistance provider to determine and calculate appropriate assistance amounts. Support participants in swiftly transitioning out of the Rapid Rehousing program while ensuring stabilization. Plan and lead group life‑skills sessions covering budgeting, financial management, housing search strategies, nutrition, tenant–landlord relationships, rights, health education, healthcare navigation, and parenting. Maintain comprehensive records, including detailed documentation and regular reports. Compile and analyze data for summary reports, ensuring compliance with Five Keys standards and Department of Homelessness and Supportive Housing (HSH) requirements. Document client feedback and resolve grievances per established protocols. Perform other duties as assigned. Administrative Duties
Enter client data—enrollments, assessments, contacts, units of service, referrals, and case notes—accurately and timely. Enter client data and outcomes into required systems (Apricot, ONE, Clarity, etc.) and report per secure email as needed. Provide monthly and quarterly reports of client activities as instructed by the Assistant Director. Create a welcoming environment for all clients and colleagues. Collaborate with community‑based organizations and government agencies to connect clients with housing, public benefits, employment services, substance use treatment, and on‑site support. Support administrative tasks related to Five Keys Rehousing Programs and provide requested information to Five Keys Administration. Maintain accurate records and log all referrals made in Clarity and Apricot. Document any protocol violations with an incident form. Abide by Five Keys and HSH policies and procedures. Provide quality assurance for programming and overall housing case management. Perform other duties as assigned. Knowledge and Skills
Experience with recovery models, including harm reduction principles. Knowledge of resources for TAY participants across education, health, housing, financial literacy, mental health, and workforce development. Comfort, maturity, and professionalism in engaging diverse partners, participants, and colleagues. Clear and calm communication with participants and partners. Advanced proficiency in Zoom, Microsoft Office, Google Workspace, and database systems. Willingness and ability to grow professionally, take accountability, and adapt to changing environments. Understanding of challenges and needs of Five Keys participants. Maintain strong, appropriate boundaries with participants and partners. Maintain strict confidentiality and handle sensitive information securely. Exceptional written communication skills. Excellent interpersonal and organizational skills, advanced problem‑solving, and tactfulness. Eagerness to learn new things and flexibility to adapt. Ability to manage multiple deadlines and tasks simultaneously. Proven case management and group facilitation skills. Proven track record in completing administrative tasks. Bilingual English/Spanish proficiency required. Education and Work Experience
High school diploma or equivalent required. Bachelor’s degree preferred. Minimum of five years providing direct service to a similar population. Typically 2–3 years of successful experience with the homeless and young adult population. Experience working with populations representative of Five Keys students preferred. Experience with the unhoused population preferred. Experience providing social services in English and Spanish required. Physical Requirements
Must wear provided PPE and adhere to safety guidelines. Frequent talking and hearing conversations. Long periods of walking, sitting at a desk and/or computer station. Lifting up to 25 lbs; reaching, bending, carrying, and stooping as part of daily routine. Additional Requirements
Submit proof of clear TB results dated within 60 days prior to hire date. Work Environment
