Program Description Under the Department of Homelessness and Supportive Housing (HSH), the Felton Institute Urgent Accommodation Voucher (UAV) Program provides emergency short-term hotel stays and supportive services for adults experiencing vehicular homelessness in San Francisco. The program’s primary goals are to connect participants with essential services, support their stabilization, and transition them from temporary hotel placement into shelter as quickly as possible.
The program will serve at least 50 adults living in vehicles, offering them access to safe, appropriate hotel accommodation while working toward shelter placement. Felton will manage hotel operations and deliver supportive services including case management, referrals to Coordinated Entry, harm reduction, benefits enrollment, health care, and behavioral health support.
The UAV Program follows a low-barrier model, ensuring individuals can quickly secure safe, temporary housing while receiving the support needed to move toward stability and longer-term housing solutions.
Job Description Under the supervision of the Program Supervisor, the Case Manager will assist the Program Supervisor in seeking appropriate hotel and/or temporary emergency shelter locations to adults experiencing vehicular homelessness throughout San Francisco. This position will provide emergency hotel placement case management and supportive services, including linkages to existing programs such as Coordinated Entry, harm reduction services, public program benefits, health services, and behavioral health referrals. The case manager will operate a referral phone line that may extend hours that include evenings and weekends to respond to emergency requests from HSH outside of normal business hours. Case Managers will assist and advocate for clients in obtaining and maintaining benefits and identify services available within the community that meet specific needs, or support progress towards identified goals.
Specific Responsibility
Provide intakes and assessments, including evaluations, service plan development, and crisis intervention to prevent hospitalization and/or homelessness.
Assist clients in accessing housing, financial, medical, and other needed services as necessary based on individual service plans.
Ensure outreach methods that include in-person interactions, phone calls, and emails, as available and appropriate to reach each participant.
Track and document wellness checks and safety checks, room inspections, case management meetings, engagement, and status of guests at least weekly to ensure clients well-being.
Assist the clients with obtaining or maintaining benefits such as cash aid, food programs, medical clinics and/or in-home support. Problem-solve when clients are disenrolled from county benefits.
Maintain and be in compliant with HSH clinical requirements and standards. Document and report services provided in HSH ONE System, and Felton CIRCE EHR system, a CARBON reporting system.
Participate in team meetings and case conferences with HSH, other collaborative agencies, and within Felton.
Commits to ongoing training provided by Felton Institute, and HSH.
Other duties as assigned.
Minimum Qualifications
A minimum of 2 years of case management and mental health treatment for at-risk adults experiencing homelessness.
Ability to work cooperatively on a multi-disciplinary team, and to work with clients with various ethnic backgrounds.
Valid CA Driver’s License required – driving is a significant part of this position.
Bi-lingual in threshold languages strongly preferred (Spanish, Cantonese, Mandarin, Vietnamese, Russian, Tagalog).
Ability to navigate and support clients in the community.
Strong communication skills, both verbal and written.
Strong interpersonal, organizational, and time management skills.
Ability to complete expected tasks within the required time frames.
Ability to prioritize projects and follow through.
Ability to be a team player with a welcoming attitude.

Salary Range: $70,000-$80,000 USD Annually (DOE)
