Overview
Provide ongoing support to individuals with intellectual and other disabilities to promote independent daily living and overall wellbeing. Responsibilities
Deliver guidance and support for self‑care routines, health, safety, household chores, socialization and overall wellbeing. Facilitate community participation and build natural supports and relationships. Maintain a safe and clean home by supporting completion of household chores and, when needed, assisting directly. Follow treatment plans and use adaptive and safety equipment to promote independence and health. Teach meal preparation skills, diet adherence, portion control and cooking concepts. Assist with medication administration according to AMAP policies and procedures. Implement and document treatment plans and agency materials to support independence and health. Adhere to overtime, scheduling and staffing policies to ensure quality support. Participate in inter‑disciplinary team meetings to discuss progress and goals. Provide transportation to medical appointments, shopping, recreational outings and other needs. Mentor new Direct Support Staff by demonstrating routines and reviewing training manuals. Participate in House Meetings and maintain certification and training expectations. Perform any other duties as requested by supervisor. Qualifications
High school diploma or equivalent preferred. Valid NYS driver’s license and qualified driver status throughout employment. Ability to communicate and understand written and oral directives and information. Training on Personal Outcome Measures (POM) required prior to evaluations and assessments. Additional Requirements
This position requires regular and substantial unsupervised or unrestricted physical contact with people receiving services and, therefore, is subject to successful completion of fingerprint/criminal background check. Physical Requirements
Regular lifting and transferring of individuals (children and/or adults). Must meet the physical demands of the position. Employment Details
