Job Summary The Nurse provides health care services to program clients, assists clinical staff with the development of treatment goals that pertain to medical issues, and acts as a liaison between agency staff and program members, attending physicians and health care providers. The nurse educates both staff and program members on health issues and assists in the development of agency policy with regard to health issues. Success is a result of innovation and creativity, flexibility, and a willingness to apply learned practices and experiences effectively. Flexibility and effective time management practices are essential. This position calls for self-confidence, creativity, and the ability to exercise individual discretion or judgment. Sound organizational skills and the ability to handle multiple taskings are a must. S/he must be able to devote the time required to ensure effective support to employees and the agency. S/he must be able to dissolve and resolve conflict within the staff and client structure. S/he must possess a high level of crisis management skill, and the ability to work through crisis situations in a professional, therapeutic and positive manner. This position is classified as a category 2 driving position.
Pay ranges from $40.00–$45.00 per hour. Position offers $2.00 shift differential for overnight shifts.
Responsibilities
Provide health care services to program clients.
Monitor client medications and evaluate effectiveness and side effects; report assessment to attending physician as necessary.
Evaluate client medical concerns, referring them for proper treatment.
Learns, understands and appropriately utilizes the agency EHR system at a level of proficiency appropriate to the role.
Schedule all entry physicals, screen processes for communicable diseases, and arrange follow-up appointments.
Coordinate medical testing, screenings and medical appointments.
Supervise and maintain all records of client medication use.
Instruct clients on responsible care of medication, hygiene, nutrition, birth control and other mental health issues as appropriate.
Work with mothers to obtain immunization records to allow children to enter childcare facilities.
Work with clients to address concerns regarding children's health and welfare and making appropriate care referrals.
Advise clinical staff of ongoing medical concerns regarding children's health and welfare and making appropriate care referrals.
Assist clinical staff with the development of treatment goals as they relate to medical issues and/or concerns.
Attend staffing and case conferences.
Educate both agency staff and clients on health care issues.
Conduct monthly awareness and prevention training on infectious diseases.
Provide speaker for weekly health lectures for program clients.
Act as liaison between agency staff and program clients, attending physicians, and/or health agency providers.
Communicate health care concerns with program clients’ attending physicians or health care providers and advise agency staff of medical care arrangements.
Coordinate program health issues with psychiatric consultants and clinical staff.
Maintain effective professional relationship with the medical community and coordinates for additional medical resources, where required.
Assist program director with program data collection, as needed.
Assist in developing agency policy with relations to health care issues and training.
Assist in the development and implementation of health-related agency policies.
Assist in developing policies on urine screening and monitoring program client medications.
Provides on call coverage to be shared with other Nursing staff.
Safeguard personally identifiable health information (PHI) in accordance with duties and current agency HIPAA notification statement and standards.
Attain, maintain and promote a positive, safe, harmonious, productive and challenging work environment.
Maintain favorable driving record.
Perform other duties as required.
Qualifications / Requirements Required
Registered Nurse with valid Minnesota State nursing license
Excellent oral and written communication skills
Previous experience with EHR/EMAR
Ability to structure time well and develop priorities
Two years free of any problems associated with chemical dependency
One year off correctional supervision
Ability to relate to people from diverse backgrounds with multiple individual and systemic barriers to success
Positive driving record
Valid state driver’s license
First Aid/CPR Certified (Certified to train preferred)
Current Mantoux test (annual requirement thereafter)
Preferred
At least 2 years of psychiatric or SUD treatment experience
RS EDEN requires all new hire candidates for this position to successfully undergo and pass a criminal history background check, and driver record screen (DMV check).
