Position Summary The Case Manager is a clinician who in conjunction with other members of the healthcare team is accountable for timely coordination of quality health care services to meet the patient's healthcare needs across the continuum of care. This position requires knowledge of the development stages as services are provided to infants, children, adolescent, pediatrics, adult, and geriatric populations. Also requires knowledge of cultural diversity and influences in the response to illness, as there is diversity in the population services. Case management initiates and implements utilization / resource management, continued stay monitoring, discharge planning, supporting optimal patient care outcomes along the continuum of care.
What It Takes to be Successful Required Qualifications
Must work every other weekend
Current California Registered Nurse license
All external candidates must have a Bachelor of Science in Nursing
Working knowledge of Intequal IS-SI Criteria
Working knowledge of Medicare, Medi-Cal, NCQA, Heidis, and other criteria identified by the Quality/Care Management Department
Minimum of 1 year discharge planning experience
Working Knowledge of managed care and capitation operations
Must possess the skill, knowledge and ability essential to the successful performance of assigned duties
Must demonstrate customer service skills appropriate to the job
Excellent written and verbal communication skills in English
Ability to maintain a work pace appropriate to the workload
Ability to establish and maintain effective working relationships across the Health System
Preferred Qualifications
Certified Case Manager Certification
Knowledge of University and medical center organizations, policies, procedures and forms
