Division Overview The Corrections Transition Programs (CTP) is a statewide network of clinical reentry management services aimed at increasing opportunities for successful reentry outcomes. CTP provides specialized case management to help individuals prepare to return to their families and communities after incarceration, offering services inside prisons and throughout communities.
Location & Compensation Central Illinois – Kankakee, Iriquiois, Ford, and Vermillion counties. Salary: $44,676 – $47,650 annually, contingent upon experience and education.
Position Summary This full‑time role provides addiction intervention, AODA counseling, and recovery support services to clients and their families. The Case Manager brings together services, agencies, and resources to assist clients in achieving their goals, working with high‑risk caseloads, co‑occurring clients, families, and children.
Essential Duties and Responsibilities
Assist individuals in achieving service plan goals through direct services and coordination of behavioral health, public agencies, and community resources.
Assess, evaluate, facilitate, and monitor individuals’ progress relative to their client‑centered service plan and parole conditions.
Complete intake packets for program enrollment and participation.
Provide referrals and crisis intervention for identified needs.
Interview individuals, review records, and confer with other professionals to assess progress and suitability for program participation.
Collaborate with internal and external partners, attend staff meetings and monthly supervisions, and integrate staffing with TASC service recipients.
Identify and refer individuals with behavioral health needs.
Prepare and submit internal and external program reports.
Maintain all required documentation: screening results, enrollments, service plans, reports, progress notes, discharge summaries, and accurate service record reporting.
Monitor individuals’ progress toward service goals.
Assist clients with enrollment in public benefit systems and obtaining vital records.
Ensure compliance with the TASC database system and contractual/external databases.
Develop effective working relationships with internal and external partners.
Participate in meetings to adjust, update, or revise client‑centered service plans.
Obtain and analyze progress reports from partners to ensure benchmarked progress.
Maintain confidentiality of all organizational, staff, and client information.
Act as an excellent steward of TASC, modeling the organization’s core values.
Our Ideal Candidate
High school diploma or GED equivalent.
Valid driver's license.
CADC certification within two years of employment (may be required based on contractual obligations).
Successful completion of an IDOC background check.
Proficiency in the Microsoft Office suite.
Experience in group facilitation.
Knowledge of ASAM placement criteria, DSM‑V, and substance abuse professional ethics (or willingness to acquire the knowledge).
Comfort working within a correctional setting and with individuals convicted of serious violent offenses.
Flexibility in thinking and approaches to leading group discussions.
Strong verbal, written, and interpersonal communication skills.
Excellent time‑management skills and a proven ability to meet deadlines.
