Job Title: Clinic LPN

Location: Helen Snell Cheel Medical Campus, Potsdam, NY
Hours Per Week: 40 scheduled weekly hours
Schedule: 8‑hour shifts from 9:00 am – 5:30 pm
Sign‑On Bonus: Up to $3,000
Summary: The licensed practical nurse performs tasks and responsibilities within the framework of casefinding, health teaching, health counseling, and the provision of supportive and restorative care under the direction of a licensed health care provider.
Responsibilities:
Collects patient data to be used by the licensed health care provider in the comprehensive care of the patient; obtains vital signs, allergies, current medications, medical history, social history, family history and chief complaint, and documents them appropriately in the electronic medical record.
Responsible for identifying problems within the practice and communicating them to their manager; assists in maintaining a clean and organized department to ensure a safe patient care environment and supports all initiatives of the health care team while promoting excellence in customer service.
Performs and documents direct patient and family‑centered care according to nursing standards of practice; prepares charts for the providers to assure that the needed information is present on each chart before the patient is seen in the office.
Provides routine nursing care and treatment to patients under the direction of the licensed health care provider; collects specimens, performs medication reconciliation and administration, and assists providers with simple procedures.
Efficiently uses medical and clerical supplies to help effectively control practice expenses; appropriately documents all treatments performed during patient encounters and adds the appropriate CPT code as required.
Collaborates professionally with all members of the health care model in the provision of patient care; works with PPM team to comply with all established standards required under the accreditation standards of DNV and is knowledgeable of state and federal health care standards.
Required Qualifications:
Graduate of a practical nursing program
Current, unrestricted New York State practical nursing license
BLS within three months of hire
Preferred Qualifications:
Displays knowledge of medications, their administration, and untoward effects.
Union: 1199‑721 – LPN & Technical (CPH)
Note: Not all per diem roles are union eligible.
Physical Requirements: Medium work – Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; requires frequent walking, standing or squatting.
For disease specific care programs, refer to the program‑specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements. Any physical requirements reported by a prospective employee and/or employee’s physician or delegate will be considered for accommodations.
Pay Range: $25.51 – $37.38
The listed base pay range is a good faith representation of current potential base pay for a successful full‑time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
