Summary:

The licensed practical nurse performing tasks and responsibilities within the framework of case finding, health teaching, health counseling, and provision of supportive and restorative care under the direction of a licensed health care provider.

Responsibilities

Collect patient data to be used by the licensed health care provider in the comprehensive care of the patient. Obtain vital signs, allergies, current medications, medical history, social history, family history, and chief complaint, and document them appropriately in the electronic medical record.

Identify problems within the practice and communicate them to management. Assist in maintaining a clean and organized department to ensure a safe patient care environment. Support all initiatives of the health care team and promote excellence in customer service. Work with the PPM team to resolve clinical issues and identify opportunities for improvement. Participate in ad‑hoc projects as assigned. Collaborate with the PPM team to address daily operational issues and solve problems to comply with the delivery of quality patient care. Integrate CPH mission and core values within the sites. Work in collaboration with CPH and PPM Departments as warranted.

Perform and document direct patient and family‑centered care according to nursing standards of practice. Prepare charts for the providers to assure that the needed information is present on each chart before the patient is seen in the office.

Provide routine nursing care and treatment to patients under the direction of the licensed health care provider, collect specimens, perform medication reconciliation and administration, and assist providers with simple procedures. Support and adhere to administrative and nursing service policies and procedures.

Efficiently use medical and clerical supplies to help control practice expenses. Appropriately document all treatments performed during patient encounters and add the appropriate CPT code as required.

Collaborate professionally with all members of the health care model in the provision of patient care. Work with PPM team to comply with all established standards required under the accreditation standards of DNV. Be knowledgeable of state and federal health care standards, as appropriate. Adhere to CPH and PPM policies, procedures, guidelines, and protocols.

Required Qualifications

Graduate of a practical nursing program.

Current, unrestricted New York State practical nursing license.

BLS within three months of hire.

Preferred Qualifications

Displays knowledge of medications, their administration, and untoward effects.

Physical Requirements Medium work: Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; requires frequent walking, standing or squatting. For disease‑specific care programs refer to the program‑specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements. Any physical requirements reported by a prospective employee and/or employee’s physician or delegate will be considered for accommodations.

Pay Range $25.51 - $37.38 per hour.

The listed base pay range is a good faith representation of current potential base pay for a successful full‑time applicant. It may be modified in the future and be eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
