Providence is offering a
RN Care Manager - PACE
role. A $15,000 hiring bonus for eligible external candidates meeting required qualifications and payment conditions.
The Registered Nurse Care Manager establishes a well‑rounded care practice that includes patient assessment, care planning, implementation, coordination, monitoring, and ongoing routine evaluation that ensures medically and financially appropriate care.
Responsibilities
Facilitate interdisciplinary team care coordination.
Conduct intensive medical management of participants recently discharged from hospital and skilled nursing facilities.
Work closely with the IDT Social Worker and Physician to collect necessary medical and social information to ensure comprehensive decisions and participant safety and well‑being.
Establish patient assessment, care planning, implementation, coordination, monitoring, and ongoing routine evaluation.
Required Qualifications
Education sufficient for clinical licensure.
Upon hire: Washington Registered Nurse License.
At least 1 year of long‑term care or geriatric nursing experience.
Preferred Qualifications
Bachelor's Degree in Nursing.
Experience in medical surgical or rehabilitation medicine.
