RN Care Manager - PACE
$15,000 hiring bonus for eligible external candidates that meet required qualifications and conditions for payment.
The Registered Nurse Care Manager establishes a well‑rounded care practice that includes a patient assessment, care planning, implementation, coordination, monitoring, and ongoing routine evaluation to ensure medically and financially appropriate care. Responsibilities include facilitating interdisciplinary team care coordination and intensive medical management of participants recently discharged from hospital and skilled nursing facilities. The position also works closely with the IDT Social Worker and Physician to collect necessary medical and social information to ensure comprehensive decisions that safeguard the safety and well‑being of participants.

Required Qualifications
Education sufficient for clinical licensure
Upon hire: Washington Registered Nurse License
1 year of long‑term care or geriatric nursing experience
Preferred Qualifications
Bachelor's Degree in Nursing
Experience in medical‑surgical or rehabilitation medicine

Location & Compensation Location: 4515 Martin Luther King Way S, Seattle, WA Job Schedule: Full time Pay Range: $51.43 – $79.84 per hour (base). Additional compensation may be available for shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonus opportunities.
