Physical Therapist 1, Case Manager: Home Health
Full‑time position in Pacifica, Half Moon Bay, and La Honda. You will evaluate and treat patients in their homes, develop care plans, supervise PTAs, and coordinate care with physicians.
Key Responsibilities
Perform home evaluations and develop individualized care plans.
Provide mobility, movement, and functional training to restore independence.
Create detailed treatment plans and update them regularly.
Communicate and coordinate with patients’ physicians and interdisciplinary team.
Document progress and report to medical directors.
Qualifications
Current unrestricted California Physical Therapist license.
BLS/CPR certification.
Valid California driver’s license.
Knowledge of local community resources.
Understanding of patient development, human behavior, culture, and state/federal home‑care regulations.
Experience in home‑health care preferred; high‑acuity patient experience a plus.
Experience with interdisciplinary teams and case‑management fundamentals.
Self‑directed, empathetic, critical thinker, and strong interpersonal and organizational skills.
Benefits & Compensation
Base salary AMD 120,000 – 135,000 per year plus annual bonus.
Mileage reimbursement $0.655 per mile.
