Available Territories, Pay Structure & Schedule We are hiring for three separate territories. Assigned a primary territory but must be willing to travel to adjacent zones when caseload demands. Compensation is based on experience and territory assignment:
Pay Rate:
$72.00 – $81.00 per hour
Annual Full-Time Expectation:
$150,000 – $170,000 per year, with eligibility of a $5,000 Sign on Bonus
Pay Rate:

Annual Full-Time Expectation:
$150,000 – $170,000 per year
TriValley Area
Pay Rate:
$72.00 – $81.00 per hour
Annual Full-Time Expectation:
$150,000 – $170,000 per year
Coverage Area:
Dublin/Livermore/Pleasanton/San Ramon/Danville
Schedule:
Flexible.
Monday – Friday standard, with rotational weekend coverage (1 weekend per month/quarter) required.

Responsibilities
Provide physical therapy services within the scope of practice and in accordance with the individualized plan of care
Deliver treatments, evaluate patient response, and promptly report any changes in condition to the Physician, Case Manager, and/or Director of Nursing
Coordinate care collaboratively with the Interdisciplinary Team to support optimal patient outcomes
Maintain accurate, timely clinical documentation and submit all required records in accordance with organizational policy using the company-issued device
Provide Moderate supervision to a Physical Therapist Assistant (PTA) when assigned, ensuring adherence to the plan of care and quality of services delivered
Demonstrate competence and confidence in managing higher-complexity cases
Follow all company policies and procedures
Perform additional duties as assigned

Requirements
Experience: Minimum 2 years of Home Health experience required. Must be proficient in OASIS and case management.
Current CA Driver’s License
