multiple territories .

Available Territories & Pay Structure We are hiring for three separate territories. Compensation is based on experience and territory assignment:

Alameda County & West Contra Costa County

Pay Rate:

$60–$70 per hour DOE - Eligibility of a $10,000 Sign on Bonus for Full-Time status employees Coverage Area:

Oakland/San Leandro/Berkeley/Richmond/San Pablo

East Contra Costa County

Pay Rate:

$60–$70 per hour DOE Coverage Area:

Antioch/Pittsburg/Brentwood

TriValley Area

Pay Rate:

$60–$70 per hour DOE Coverage Area:

Dublin/Livermore/Pleasanton/San Ramon/Danville

Responsibilities

Provide Occupational Therapy services according to the individualized plan of care

Deliver treatments, evaluate patient response, and report condition changes to the Case Manager, Physician, and/or Director of Nursing

Maintain accurate and timely clinical documentation

Adhere to all company policies and procedures

Collaborate effectively with the interdisciplinary team to ensure optimal patient outcomes

Requirements

Home Health Experience: 1 year (Preferred)

Current CA Driver’s License

Excellent organizational and communication skills

Ability to work independently in the field
