Talented, self‑directed Occupational Therapists are needed for home‑health visits throughout the San Francisco Bay Area, including San Francisco, Oakland, and surrounding counties. The position is part‑time with flexible scheduling and full home‑visit coverage.
Pay range:
$120,000.00/yr – $130,000.00/yr (base pay, with incentive bonus and full benefits).

Responsibilities
Partner with the Case Manager to assess and address patients’ needs in the home setting, promoting safety and productivity.
Provide services in home safety, medication management, wheelchair seating and positioning, pain management, falls prevention, chronic disease management, dementia care, and family caregiver training.
Educate patients and families, advocate for patient needs, and collaborate with the interdisciplinary care team.
Document care and progress in electronic health records and support quality metrics.

Qualifications
Current unrestricted California Occupational Therapist license.
Current BLS/CPR certification.
Valid California driver’s license.
Knowledge of local community resources.
Understanding of individual development and human behavior related to illness/injury, and influence of culture on health care.
Knowledge of state and federal home‑care regulatory guidelines (preferred).
Experience in home health care (highly preferred).
Experience with high‑acuity patients (plus).
Experience working with an interdisciplinary team (highly desired).
Case management fundamentals (highly desired).
Self‑directed, empathetic, critical thinker, outstanding interpersonal skills, leadership, organized.
Completion of a computer literacy and competency assessment prior to in‑person interview.
