The Home Health Occupational Therapy Care Manager provides independent management of home care patients requiring OT, coordinating assessments, planning, and interventions based on individual needs.
Responsibilities include evaluating patient function, developing personalized care plans, educating patients and families, and monitoring outcomes. Qualifications include a Master’s in Occupational Therapy, 1 year of relevant experience, active BLS, and a valid driver’s license. Preferred candidates have experience in Medicare-certified home health, case management certification, and OASIS specialization.
