Summary / Objective Responsible for the direction and administration of the Resident Assessment Instrument (RAI) process to ensure accurate and timely completion of resident assessments in accordance with Medicare, Medicaid, OBRA, and other payor program requirements. The role ensures assessments accurately reflect each resident’s physical, mental and psychosocial status, optimize reimbursement, and support documentation needs.

Essential Functions

Oversee and coordinate the development and completion of the resident assessment (MDS) in accordance with federal and state regulations.

Coordinate and lead the development of a comprehensive resident assessment and care plan for each resident as chairperson of the center Interdisciplinary Plan of Care team.

Ensure face validity of all Minimum Data Sets before electronic submission.

Combine the timely electronic submission of all MDS.

Conduct monthly reviews of random MDS samples to ensure accuracy and internal consistency with the medical record.

Coordinate the completion of each resident’s MDS in compliance with all state and federal requirements.

Calculate triggers and develop resident assessment protocols for initiation of care plans.

Ensure MDSs are completed timely, accurately identify resident level of functioning per RAI manual guidelines, and maximize reimbursement opportunities within legal limits.

Train nursing staff on responsibilities for timely and accurate clinical record documentation.

Review and assess all resident information (including hospital records) to accurately complete MDS scoring.

Maintain MDS and supporting clinical record documentation according to state and federal requirements and center policies.

Coordinate and assist with weekly interdisciplinary care conferences, Medicare meetings, and CTO referrals to nursing and therapy departments.

Development and Implementation

Correct pending Medicare and therapy billing errors related to MDS.

Generate MDS audit reports as needed; verify Medicaid billing accuracy; maintain accurate billing for HMO and VA residents.

Compile monthly and quarterly Medicare/Medicaid default reports; ensure submission and processing of MDS reports via CASPER, Validation Reports, etc.

Identify and correct MDS/Census and AR errors for center billing information.

Provide training on clinical software and Care Tracker.

Participation and Compliance

Participate in interdisciplinary care team.

Stay current with state and federal regulatory changes.

Comply with laws and regulations applicable to the position and act in accordance with the Center’s Compliance Program.

Attend in-service training, performance improvement committees, and related meetings as scheduled.

Safety and Resident Rights

Follow center rules, dress and hygiene policies, and smoking policies.

Demonstrate proper equipment use and report needs or repairs.

Report incidents or accidents, hazardous conditions, and protective equipment usage.

Follow infection control standards, policies and procedures.

Know and help residents exercise or protect their rights; report complaints; maintain confidentiality of resident information.

Follow and adhere to the Center’s HIPAA policies; use the minimum amount of PHI necessary; report suspected violations.

Required Education and Experience

Registered Nurse (RN) in good standing with the state licensing board.

Clinical experience and education or specialty skills specific to geriatrics.

Demonstrated competence in PPS assessment and OBRA compliance.

Excellent clinical assessment, organization, and independent decision-making skills.

Knowledge of state and federal regulations governing the center’s practices.

Excellent interpersonal skills.

Computer proficiency with Microsoft products.

Preferred Education and Experience

2+ years of previous experience in a fast‑paced environment.

Post‑acute care or general health care experience preferred.

Skills, Knowledge, and Abilities Experience with MDS software and Microsoft Office. Strong oral and written communication, organization, and project management skills. Ability to work with all levels of employees and effectively relate with residents, families, and staff. Working knowledge of the MDS and RAI process, Medicare, Medicaid, and other payor reimbursement systems. Basic computer skills and aptitude for learning company applications.
