Chief IV Registered Nurse Community Care – Manchester VA location. Full‑time 40 hours per week.
Qualifications
English Language Proficiency – may be complete deficit or per 38 U.S.C. 7403(f)
Graduate of a nursing school accredited by ACEN or CCNE, or equivalent
Masters bridge program completion: may become registered prior to graduation
State‑licensed nurse; temporary appointment and later permanent upon successful bridge program completion (Reference: VA Handbook 5005 – Appendix G6)
Foreign school graduate with current full active unrestricted registration eligible for appointment
Foreign nursing education above associate level requires foreign equivalency validation (ICD)
Current full active unrestricted registration in any U.S. State, Territory or Commonwealth
Graduate Nurse Technician (GNT) – temporary 120‑day appointment while pursuing licensure, extendable up to two years
Grandfathering Provision: All persons in VHA 0610 series meeting duties on 1/29/2024 are considered qualified
Preferred Experience: Leadership and supervision for clinical and administrative nursing programs
Personnel/FTEE management, staff education, budgeting and rapid growth initiatives
Leading collaborative initiatives across services to influence mission and healthcare
In‑depth understanding of healthcare systems and community care programs
Ensuring adherence to performance measures and compliance with internal and external standards
Ability to assess needs and support others via high‑professional collaboration
Scope – Education and Dimension criteria for Nurse IV details
Education: MSN and 4 years experience with specialized nursing experience at Nurse III level; or related field with BSN; or doctoral degree and 3 years experience

Duties
Chief of Community Care – executive leadership, direct supervision of Office of Community Care
Clinical and administrative coordinator for all aspects of Community Care, including policies, budget, compliance with VHA Directives
Direct responsibility for administration and clinical operations and personnel within Office of Community Care
Leads populations or integrated programs, coordinates cross‑discipline areas, influences mission, facilitates care coordination
Manages all Community Care consults and requests for additional services as directed
Sound clinical and administrative decision‑making to enhance profession
Participates in multidisciplinary activities involving treatment plans and discharge plans for bed‑service patients
Utilizes problem‑solving methodologies to resolve patient‑care impacts
Review clinical and administrative eligibility for Community Care services
Guides all staff assigned to Office of Community Care, assists training and stakeholder use of Community Care
Coordinating care with outside providers and communicating resources and specialty care availability
Facilitates ongoing communication between staff and leadership, evaluates data, reviews workload demands
Fosters team‑oriented environment among staff and collaboration with administrative and clinical staff
Participates in medical‑center committees as assigned
Revises schedules and assignments on short notice to meet changing workload demands

Other responsibilities: handle financial disclosure reporting, SF‑50 submission, relocation and recruitment incentives, etc.
