Responsibilities Powerback provides a new way of delivering recovery, rehabilitation, respiratory, and wellness services to patients across the nation. This role primarily offers outpatient rehabilitation services in an in‑home setting for the older adult population. The Physical Therapist evaluates and treats in the most functional environment—the patient's home—designing and delivering individualized therapy programs to facilitate rehabilitation, promoting independence, and ensuring patient safety.
Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel in accordance with state licensure requirements and professional standards.
Adhere to department and patient treatment schedules, timely clinical and billing documentation, and work‑related meetings.
Attend and contribute to patient care, staffing, and other related meetings.
Make referrals to other disciplines and agencies to address comprehensive patient needs.
Consult with rehabilitation team members to select appropriate therapeutic regimens.
Adapt program and/or plan of care based on individual patient needs.
Promote a safe environment and enforce elimination of fire and safety hazards.
Order supplies and equipment as necessary.
Clean, maintain, identify, and report equipment repairs.
Put patient service first: ensure patients and families receive the highest quality service in a caring, compassionate atmosphere that recognizes individual needs and rights.
Screen, examine, and evaluate patients, including history, systems review, and application of appropriate tests and measures. Synthesize information and interpret findings to establish a diagnosis, identify impairments, determine predicted improvement, and design a plan of care. Develop treatment goals and methods collaboratively with patients and caregivers, implement the program, and complete all documentation. Services include: Therapeutic exercise— aerobic capacity conditioning, balance coordination, agility training, body mechanics, postural stabilization, flexibility exercises, gait and locomotion training, neuromotor development training, relaxation, strength, power, and endurance training
Functional training

Airway clearance techniques
Integumentary repair and protection
Use of electrotherapeutic, physical agent, and mechanical modalities
Health and wellness
Education on chronic disease management
Falls assessment and interventions to reduce fall risk
Case management in home and community
Design, fabricate, fit, and apply adaptive devices, and train patients on their use, including orthotics and prosthetics.
Select activities suited to individual physical, mental capacity, intelligence level, and interest.
Instruct patients in skills and techniques for mobility activities, evaluating progress, attitudes, and behavior related to rehabilitative potential.
Perform routine reexamination as needed to modify or progress treatment plan.
Incorporate health and wellness programming into treatment plans.
Perform home and community mobility assessments and provide recommendations for safe mobility.
Perform other related duties as required.
Qualifications
Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.
Must have a Master’s degree in Physical Therapy.
Or must have a Master’s degree in a Physical Therapy‑related field in addition to a Bachelor’s degree in Physical Therapy.
Or must have a Bachelor’s degree in Physical Therapy plus 5 years of progressively responsible PT experience.
The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated as the U.S. equivalent to a degree in Physical Therapy.
Must have a valid driver’s license or reliable, independent transportation to and from the patient’s residence.
Salary Range USD $66.00 – $66.00 /hr.
