Responsibilities
Deliver exceptional care to clients in their homes
Execute comprehensive personalized weekly schedules and care plans
Provide skilled nursing services per client’s plan of care within scope of practice
Counsel clients and families to meet nursing and related needs
Ensure documentation meets agency policy and procedures
Coordinate services by participating in case conferences, case management, team meetings, and staff meetings
Perform administrative tasks such as documenting completed tasks and reporting changes in client’s physical condition, mental capability or behavior
Qualifications
Registered Nurse (RN) license required
BLS certification required
Experience placing peripheral IVs, accessing ports, and working with PICC lines
Sound judgment and ability to remain calm in varied situations
Basic computer skills
Proof of eligibility to work in the United States
