Role Summary We’re seeking an experienced Clinical Specialist to provide advanced clinical oversight and leadership within our community-based aged care services. In this role, you’ll deliver comprehensive assessments, care planning, and strong clinical governance for older people receiving support under the new Support at Home program. You’ll champion safe, high-quality, personalised and goal-directed care aligned with the Aged Care Act, Aged Care Quality Standards and the strengthened regulatory framework. Acting as a clinical leader, you’ll support care teams, consumers, families and organisational leadership to deliver evidence-based, person-centred care that empowers older people to live safely, independently and with dignity at home.
Responsibilities and Duties
Conduct comprehensive clinical assessments and develop personalised care plans for older people
Provide expert clinical guidance for complex care needs, including chronic disease, falls, wounds, cognition, and medication management
Ensure compliance with Aged Care Quality Standards and maintain strong clinical governance
Support, mentor, and educate care teams, fostering evidence‑based, person‑centred practice
Build culturally safe, respectful relationships with consumers and families, advocating for their rights
Collaborate with multidisciplinary teams to deliver safe, coordinated, and integrated care
Identify and manage risks, safeguarding concerns, and meet all mandatory reporting obligations
This role is being hired on a permanent full‑time basis and is located out of our central office in Belmont.
Skills, Attributes and Experience
Valid AHPRA Registration
Must hold a valid driver’s licence and have access to a reliable vehicle
Possess or be willing to obtain a NDIS Worker Screening Check and National Police Clearance
Strong communication, time management, and relationship‑building skills
Knowledge of the Aged Care Act and Strengthened Aged Care Quality Standards
Experience in home and community care settings
Expertise in person‑centred care, clinical judgement, risk management, and compliance
Skilled in problem solving, critical thinking, education, mentorship, and cultural safety/trauma‑informed practice
Proficient in documentation and maintaining clinical governance standards
