All opportunities at 21HHS require being in the field visiting patients in their homes. Territory: Solano County: Vallejo, American Canyon, Hercules and minimal visits to Napa County. Position: Home Health Speech Therapist/Language Pathologist (ST)
Responsibilities
In partnership with the Case Manager, provide skilled services to assist patients with breathing, speaking, and swallowing functions. Evaluate and treat speech, language, cognitive and fluency disorders that affect communication. Serve as an educator, patient advocate and a member of the interdisciplinary care team. Current unrestricted CA Speech Therapist License. Current BLS/CPR Certification. Valid CA Driver's License. Knowledge and understanding of individual development and human behavior as it relates to the effects of illness/injury, and of the influence of culture on health care. Knowledge of state and federal homecare regulatory guidelines preferred. Experience in home health care highly preferred. Experience with high acuity patients is a plus. Experience working with an interdisciplinary team highly desired. Case Management fundamentals highly desired. Self‑directed, empathetic, critical thinker, outstanding interpersonal and leadership skills, organized.
