Territory: Solano County – Vallejo, American Canyon, Hercules and minimal visits to Napa County*.

What you’ll do as a Home Health Speech Therapist/Language Pathologies (ST) In partnership with the Case Manager you will provide skilled services to assist patients with breathing, speaking, and swallowing functions. You will evaluate and treat speech, language, cognitive and fluency disorders that affect communication. You will serve as an educator, patient advocate and a member of the interdisciplinary care team.

To be a success as Speech Therapist/Language Pathologist (ST)

Current unrestricted CA Speech Therapist License

Current BLS/CPR Certification

Valid CA Driver's License

Knowledge and understanding of individual development and human behavior as it relates to the effects of illness/injury, and of the influence of culture on health care

Knowledge of state and federal homecare regulatory guidelines preferred

Experience in home health care is highly preferred

Experience with high acuity patients is a plus

Experience working with an interdisciplinary team is highly desired

Case Management fundamentals is highly desired

Prior to scheduling an in‑person interview, all candidates are required to complete a computer literacy and competency assessment. $110,000 - $125,000 a year

Anticipated wage range: Speech Therapist $110,000.00 - $125,000.00. Starting pay is commensurate with relevant experience above the minimum requirements. 21st Century offers generous bonus incentive plans, comprehensive health benefits, and a 401k (up to 4% match).
